Subclinical hypothyroidism β what it means
Some people have a mildly elevated TSH with normal Free T4 levels and no clear symptoms. This is called subclinical hypothyroidism. Whether to treat it depends on the degree of TSH elevation, the presence of thyroid antibodies, symptoms, age, pregnancy status, and other factors. It does not automatically require medication. What causes hypothyroidism?
- π‘οΈ Hashimoto's thyroiditis β the most common cause in the United States An autoimmune condition in which the immune system attacks the thyroid gland over time, gradually impairing its ability to produce enough hormone. It often runs in families and is far more common in women. Many people with Hashimoto's have elevated thyroid antibodies (TPO antibodies) in their blood. - π Prior thyroid treatment Radioactive iodine treatment for hyperthyroidism, surgical removal of part or all of the thyroid, and radiation therapy to the neck area can all result in hypothyroidism, either immediately or years later.
- π Medications Several commonly used medications can affect thyroid function, including lithium (used for bipolar disorder), amiodarone (a heart medication), and certain immunotherapy drugs. If you started a new medication around the time your symptoms appeared, mention it to your provider.
- π€± Postpartum thyroiditis Some people develop temporary thyroid dysfunction in the months after childbirth. It may involve an overactive phase, an underactive phase, either phase alone, or both. Hypothyroidism and pregnancy
Thyroid hormone is essential for fetal brain development during pregnancy. Unmanaged hypothyroidism is associated with serious risks including miscarriage, preterm birth, and developmental concerns. If you take levothyroxine and learn that you are pregnant, contact your obstetric or thyroid clinician promptly. Do not wait for a routine appointment and do not stop the medication. Thyroid evaluation starts with a blood test. The tests needed depend on whether the concern is an underactive thyroid, an overactive thyroid, pregnancy, medication effects, or another clinical situation. Biotin supplements can interfere with thyroid test results
High-dose biotin supplements are commonly taken for hair, skin, and nail health and can interfere with some thyroid laboratory assays, producing falsely abnormal results. Tell your provider and the laboratory if you take biotin before having thyroid labs drawn. Do not stop biotin without instructions, as the timing depends on the specific test being used.

Some antacids and cholesterol medications also require separation. Coffee and food can reduce absorption when taken too soon after the dose. Desiccated thyroid extract has not undergone the same FDA regulatory process as approved levothyroxine products. Its relatively high T3 content can produce excessive hormone exposure in some patients, contributing to palpitations, abnormal heart rhythm, and bone loss over time. It is not appropriate during pregnancy. Whether any alternative thyroid-hormone regimen is reasonable for a given patient requires careful clinical assessment and monitoring, often with endocrinology involvement.

Supplements marketed for "thyroid support," including those containing iodine, selenium, ashwagandha, or bladderwrack, are widely promoted but not well-evidenced for treating hypothyroidism. Some can interfere with thyroid function testing or interact with levothyroxine. If you are taking any thyroid-related supplements, tell your provider so they can be factored into your evaluation. What is Hashimoto's thyroiditis and does it need to be treated differently?
Hashimoto's thyroiditis is the most common cause of hypothyroidism in the United States. The immune system produces antibodies that gradually damage the thyroid gland over time, reducing its ability to produce enough hormone. When Hashimoto's leads to hypothyroidism, the treatment is the same as for other causes: levothyroxine to replace the missing hormone. Having Hashimoto's antibodies without hypothyroidism does not automatically require medication, but it does warrant monitoring over time since thyroid function can change. A gluten-free diet is sometimes discussed in the context of Hashimoto's, particularly for patients who also have celiac disease, but evidence does not support dietary changes as a way to alter antibody levels or thyroid function in people without celiac disease.

Why does it matter when I take my thyroid medication?

Is hypothyroidism a lifelong condition?
For most people, yes. Hypothyroidism caused by Hashimoto's thyroiditis, prior thyroid surgery, or radioactive iodine treatment is typically permanent and requires lifelong medication. Some causes, including postpartum thyroiditis and medication-induced hypothyroidism, can be temporary, and medication may not always be needed long-term. Do not stop levothyroxine without discussing it with your provider first, even if you are feeling well. Feeling well is often a sign the medication is working.
Can I take iodine supplements to support my thyroid?
Most nonpregnant adults in the United States receive adequate iodine through food and iodized salt, and additional high-dose iodine does not treat hypothyroidism. For people with Hashimoto's in particular, excess iodine can worsen thyroid dysfunction rather than help it. High-dose iodine products such as kelp and bladderwrack supplements should not be taken without clinical guidance. Pregnancy and breastfeeding do increase iodine requirements, and patients who are pregnant, planning pregnancy, or breastfeeding should follow their obstetric clinician's or endocrinologist's prenatal guidance rather than starting or stopping iodine supplements on their own. Ordinary prenatal vitamins and the recommended dietary amounts during pregnancy are a different matter from the high-dose "thyroid support" products marketed online.
